Capital asset requests
To request capital funding for your clinic, you must submit a Capital Asset Request form (available from our Forms page) to the Technical and Business Support Coordinator via fax on 9341 1355 or by email to jason.dawson@dhsv.org.au.
If your clinic is being severely impacted and requires urgent works, you may instead submit the Capital Asset Request (URGENT) form.
Capital Requests will be reviewed by the DHSV Capital Review Panel.
